Step 5 – Determine whether a hazard was involved

The next step is to determine whether a hazard contributed to the event.  At this point:

• Carry out an investigation as to what has occurred.
• Conduct a hazard analysis using the Risk Calculator.
• Record the hazard in the Hazard Register.
